    Provided: Transportation, Nutrition, Case Management, Counseling, Supportive Services, Activities.

    Respite: provided for caregivers and minimizes social isolation as well as the loneliness experienced by many elderly.

    Eligibility: Men and women fifty five years of age or older who are limited by age or ill health and who can benefit from the program of maintenance, supervision and/or supportive care.
